Lifeline Verification for Utilities

Pursuant to the Commission's May 1, 2009 Order in Docket Number 08-659, eligible telecommunications carriers (ETCs) are required to send two separate notices to their surveyed customers informing them of the verification process.   The Department of Commerce has developed two customer letters which carriers are required to use in conjunction with the customer verification form. 

Reporting Sampling Requirements

Carriers shall randomly sample Lifeline recipients. Carrier size is defined by the number of lifeline recipients. The Reporting Sampling Requirements refers to the number of returned surveys rather than the number distributed. The 2011 Minnesota Lifeline Survey report must be eFiled by September 1, 2011 under docket P999/M-11-212.
