Annual Report Forms
All telephone companies that had authority (Including Conditional) in any given year, are required to file this Annual Report regardless of the revenue amount. Please submit the completed annual report to the Department of Commerce via the electronic filing system .
In order to submit a document using the eFiling application, you must be a registered user. To register, complete the application for registration on the eFiling home page and allow 24 hours to receive a user ID and password.
For information on how to use the eFiling system, please contact Jan Mottaz at 651.296.7100 Jan.Mottaz@state.mn.us . When filing, you only need to use the last two digits of the year and the docket number, (for example, 10-03). File the Annual Report as an EXCEL document only.
